Organization Description:

Founded in 2016, Focused Interruption was created by a small group of community members, many of whom have been directly impacted by gun violence and the criminal justice system. These volunteers were committed to taking action to end the growing gun violence in Dane County. Over five years, Focused Interruption has provided counseling and support to over 300 victims and perpetrators of gun violence, visiting area hospitals at all hours of night and day partnered with the City of Madison to address gun violence.

Our mission is to use a holistic approach to reduce generational trauma by providing evidence-based intervention and prevention services to the people, neighborhoods, and families most impacted by gun violence.

Position Summary:

The Aftercare and Recovery Manager is a key position within Focused Interruption, responsible for establishing and managing the aftercare and recovery program for gunshot wound patients discharged from hospitals within Dane County. This role involves collaborating with the hospitals, coordinating essential services, engaging with patients and their families, and ensuring comprehensive support through evidence-informed assessment tools. The Aftercare and Recovery Manager will actively involve patients or clients in developing personalized care plans, monitor their progress at regular intervals, and modify plans based on participants' evolving needs and circumstances.

Key Duties and Responsibilities:

1. Program Development and Collaboration:

o Collaborate with Dane County hospitals related to trauma to design and establish an aftercare and recovery program for gunshot wound patients.

o Coordinate with healthcare professionals and other stakeholders to ensure seamless transitions for patients from hospital to aftercare services.

2. Comprehensive Support Services:

o Coordinate a comprehensive array of lifesaving and lifechanging services and approaches for patients, families, and neighborhoods affected by gun violence.

o Utilize evidence-informed assessment tools to identify specific needs and develop tailored support plans.

3. Patient Involvement and Progress Monitoring:

o Involve patients or clients in the development of individualized plans of care, setting goals, and defining timelines for recovery.

o Monitor participants' progress at 3- and 6-month intervals and annually, allowing for modifications to plans based on evolving circumstances and participants' realities.

4. Community Engagement and Collaborative Initiatives:

o Collaborate with CSWs, Outreach Workers, and community members to address safety priorities and enhance protective factors within at-risk neighborhoods.

o Identify resource and policy needs to support gun violence victims and their families.

o Engage with community leaders, hospitals, and other institutions to implement strategies addressing social determinants of health (SDOH), reducing violence, and addressing racial and health disparities.

Qualifications:

Bachelor’s or Master’s degree in social work, public health, a related field, or relative years of experience.
Proven experience in program development and management, preferably in healthcare, trauma support, or violence prevention.
Strong understanding of evidence-informed assessment tools and their application in developing tailored support plans.
Excellent interpersonal skills with the ability to engage effectively with patients, families, and diverse community stakeholders.
Experience in monitoring and evaluating participant progress, adapting support plans as needed.
Ability to collaborate with multidisciplinary teams and community partners to address social determinants of health and promote community well-being.
Commitment to trauma-informed care, equity, and social justice principles.

This position is a full-time, exempt position based in Madison, Wisconsin. Focused Interruption does not provide work visa sponsorship or relocation assistance for this position.
